KEY RESPONSIBILITIES
Community Engagement & Donor Relations
  • Develop and implement strategies to engage individuals, corporations, foundations, and family offices.

REQUIREMENTS
  •  degree in Marketing, Mass Communications, Public Relations, or a related field, or equivalent practical experience.
  • 3-5 years of relevant managerial experience in community engagement, fundraising, communications, stakeholder management, or related fields (e.g., non-profit, public sector, or corporate roles).
  • Strong stakeholder management and relationship-building skills.
  • Excellent written and verbal communication abilities.
  • Proficiency in CRM systems, digital tools, and social media platforms.
  • Resourceful, adaptable, and aligned with the organisation’s mission and values.
  • Candidates with more extensive experience may be considered for the Senior Manager role.

About the Role

Description

Community Resource, Engagement and Support Team (CREST), serves as a community touchpoint for persons with or who are at risk of mental health conditions (including dementia and their caregivers). CREST conducts outreach events/activities to promote mental health awareness as well as identify targeted cases for those who require more support for their mental health needs.

The Social Work Associate is a team player with daily administrative and operational support for the purpose of CREST service development and implementation. He/ She will be involved in coordinating outreach events and activities and take on a load of cases suitable to their developmental level.

Roles and Duties

A)  Information and Support

·      Provide dementia/mental health awareness and information on managing dementia, coping strategies for all clients reached out, including those identified to be at risk, and their caregivers.

·      Provide basic emotional support to all clients or caregivers of persons with dementia/mental health conditions or who are at risk of dementia/mental health issues.

B)   Case Management

·      Assist Social Workers in monitoring assigned clients and to follow-up on client needs until the handover is completed.

·      Coordinate care support to client where necessary.

·      In-charge of timely data collation and quarterly report submission, as well as generating reports, in accordance with the CREST programme.

·      Store and maintain data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.

C)   Outreach to Community

·      Provide administrative or logistical support to the team in running client-related support and/or other CREST activities.

·      Assist in coordinating and planning outreach and engagement events with other community partners and agencies.

·      Conduct screening for clients referred from other service partners and those at risk within the community with an agreed upon identification tool (GAD/PHQ/AMT/EBAS & etc.).

D)   Others

·    Prepare the necessary resource material to be provided to clients and caregivers.

·    Undertake any other responsibilities and duties as assigned.

Requirements

·      Possess a diploma certification or relevant social service certification.

·      Candidates without a diploma but with relevant experience may also be considered.

·      Proficient in spoken and written English for client engagement, report writing, and coordination with stakeholders.

·      Ability to communicate in additional languages or dialects may be useful for engaging with diverse community groups.

·      Proactive and responsive in client interactions and outreach efforts

·      Passionate to work in the mental health sector and community service sector.

·      Keen interest in working with people on one-to-one basis or centre-based group activity.

REACH Family Service is a community based social service agency commissioned and funded by the Ministry of Social and Family Development. The role of the Social Worker is to provide support for low-income and/or vulnerable individuals and families with social and emotional issues, and work towards stability, self-reliance and social mobility.

Key Responsibilities:

  • Provide casework intervention, counselling and information & referral to families in line with REACH FSC Guidelines and MSF’S Services and Practice Standards for FSC.
  • Attend to callers/walk-ins and refer clients to appropriate agencies.
  • Maintain timely and detailed case recordings.
  • Ensure KPIs and standards of C&C are met.
  • Participate in supervision sessions and maintain proper recording of supervision, especially for risk cases.
  • Undertake supervision of interns, if assigned.
  • Develop, plan, lead, and evaluate evidence-based programmes/ group work with research components.
  • Network and collaborate with other organization/community stakeholders in planning and implementation of programme/group work.
  • Undertake additional responsibilities such as committee work for agency initiatives.

About the Role
Join our team at REACH Senior Service’s Senior Care Centre and make a meaningful impact on seniors’ lives. As a Physiotherapist, you will support rehabilitation and wellness programmes that help seniors maintain mobility, strength, and independence through safe, evidence-based physiotherapy interventions.

Key Responsibilities

  • Conduct basic physiotherapy assessments under supervision.
  • Implement and monitor individualised exercise and rehabilitation programmes.
  • Facilitate therapy sessions, group exercises, and mobility training.
  • Maintain physiotherapy equipment and assistive devices.
  • Observe, record, and report clients’ progress to the supervising Physiotherapist.
