We are looking for a committed Health Management Executive (‘HME’) who is passionate to reach out to the seniors to conduct community screening and outreach, monitor their health and enrol them into the health management plans.

Key Responsibilities:

  • Provide health awareness and basic monitoring for seniors with chronic illnesses, including those identified to be at-risk and their caregivers.
  • Perform basic health monitoring and keep track of seniors’ health plans and flag out non-compliance or other concerns to social workers / senior health management executives.
  • Plan and conduct outreach to the community to promote wellness and health awareness.
  • Manage volunteer and befrienders deployment and feedback.
  • Perform community health screenings and strengthen collaboration with local organizations.
  • Members recruitment and renewal.
  • Review and submit Community Screening Tool (CST) forms.

Requirements

  • Minimum Higher Diploma or Diploma certification in Gerontology or ITE NITEC in Community Care and Social Services, with 2 years of relevant experience.
  • Resourceful and able to do public speaking when handling events and projects.
  • Conversant in English and any 2nd language with ability to converse in local dialect would be an advantage for elderly clients
  • Competency in Microsoft Office applications especially in MS PowerPoint and MS Publisher

The Social Worker/Youth Worker for the Youth Service plays a pivotal role in driving positive change for individuals, families, and communities facing transitional challenges. This position involves providing direct services to clients and facilitating rehabilitation and reintegration efforts.

The primary scope of this role focuses on the Reformative Training Centre (Singapore Prison Service), with opportunities to engage in community outreach programs, the Singapore Boys’ Home, and other initiatives assigned by the Head of Department.

The Social Worker/Youth Worker will be responsible for developing and implementing intervention plans based on professional knowledge, conducting casework, and leading various community development activities.

Key Responsibilities:

  • Build and maintain professional relationships with clients and their families/caregivers.
  • Conduct intake assessments to identify client needs and develop tailored intervention plans.
  • Manage case plans, including regular reviews, follow-ups, and accurate documentation.
  • Facilitate group programs, including psycho-educational and therapeutic sessions.

Requirements

  • A minimum of a graduate diploma or degree in Social Work, Counselling, Psychology, Sociology, or a related field. 
  • At least two years of case management experience, with a focus on at-risk youth, is highly preferred.
  • Ability to work within time-based programs to achieve desired outcomes.
  • Self-motivated, able to work independently, and an effective team player.
  • Excellent interpersonal and communication skills for working with youth-at-risk, young offenders, and their families.
  • Skilled in case assessment, engagement, intervention planning, and prompt reporting.

REACH Family Service is a community based social service agency commissioned and funded by the Ministry of Social and Family Development. We serve vulnerable and low-income families and individuals residing in the Bishan, Sin Ming, Shunfu and Thomson vicinity.   • Maintain timely and detailed case recordings.
  • Ensure KPIs and standards of C&C are met.
  • Participate in supervision sessions and maintain proper recording of supervision, especially for risk cases.
  • Undertake supervision of interns, if assigned.
  • Develop, plan, lead, and evaluate evidence-based programmes/ group work with research components.
  • Network and collaborate with other organization/community stakeholders in planning and implementation of programme/group work.
  • Undertake additional responsibilities such as committee work for agency initiatives.

 

Requirements

  • Possess a Degree in Social Work or Graduate Diploma in Social Work
  • Minimum 2 years of relevant experience

The Community Resource, Engagement and Support Team (CREST) of REACH Community Services serves as a first mental health touchpoint in the community.  We focus on raising public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals, provides emotional support to individuals and their caregivers and links individuals to relevant health and social care services when necessary. Actively, we conduct outreach events/activities to promote mental health awareness and identify and support those with mental health needs and their caregivers.

The Social Work Associate provides the team with daily administrative and operational support for the purpose of CREST service development and implementation. You will attend to all enquiries (phone, walk in), organize the logistics &/or resources to ensure the smooth running of CREST services.   • Provide administrative and logistical support to the team in running client-related support and/or other CREST activities.

We are seeking for an energetic individual to provide the full spectrum of human resources to assist and support the HR manager in in managing the HR operations, formulating and executing plans, and ensuring quality in all deliverables.

He/ She is expected to provide advisory and support to all departments on HR-related matters and implement HR programs and initiatives. Participates and spearheads HR projects as and when required. We are looking for a passionate Programme Executive to plan and coordinate the day-to-day programme. guide volunteers to carry out the programme activities, work with other organisations to execute programmes, manage communications and feedback from stakeholders, as well as conduct outreach activities to other organisations and the community.

Key Responsibilities:

  • Plan, develop and manage engagement programmes (workshops, educational talks, festive events, etc.) to be held at designated neighbourhood touchpoints for Seniors.
  • Allocate appropriate resources, recruitment of seniors / volunteers / partners for the programmes, and to plan and execute publicity for such events.
  • Supervise and train volunteers / partners in the running of approved programmes and provide administrative support to volunteers / partners involved.
  • Monitor seniors' health and wellness, make referrals to appropriate agencies, and conduct home or hospital visits.
  • Manage the AAS (Alert Alarm System), SC phone and its maintenance.
  • Provide support for seniors, including health monitoring and communication with next of kin.
  • Conduct community outreach within the vicinity of the service area and perform needs assessment to ascertain service needs and gaps of the senior residents.

Requirements

  • Possess Higher Diploma or Diploma in Gerontology, ITE NITEC in Community Care and Social Services, or Diploma in Social Enterprise/Entrepreneurship
  • Ability to train volunteers to facilitate creative arts / baking / cooking / singing / music instruments / florist / live streaming in senior programmes.
  • Ability to host or deliver programmes in front of a crowd of 10 – 80 seniors is a must.
  • Familiarity with live-stream or data-entry and administration is preferred.
  • Experience in social services not necessary
  • Fully conversant and able to translate in Malay / Mandarin and English, ability to converse in Hokkien / Teochew will be a bonus, due to clients being seniors.
  • Must be a good team-player, with a passion to work with seniors.
