Informed Consent Form

Dear Sir/Madam

Welcome to REACH Counselling & Mental Health Service (RCMH), a professional counselling service, established in 2009 under the umbrella of REACH Community Services Ltd.. RCMH provides individual, couple and family counselling. All of our Counsellors are professionally trained and certified. Our aim is to build enduring marriages and resilient families and individuals in our community.

We are glad that you have chosen counselling as an alternative and your counsellor will work with you in managing your issues and difficulties. By clicking the box, I am indicating that I have read, understood and accepted the terms and conditions in the Informed Consent Form of REACH Community Services Ltd. as stated below:

(A) CANCELLATION POLICY

There is no refund for no-shows or cancellations of appointments. However, client can reschedule their appointments if they cannot make it more than 24 hours before the scheduled appointment date and time.

(B) CONFIDENTIALITY

All information provided in the counselling session(s) are kept confidential and in accordance with the Personal Data Protection Act (PDPA) as detailed in our Privacy Notice published on REACH Community Services Ltd.’s website; with the following exceptions:

  1. When the counsellor makes an assessment that there is foreseeable suicidal risk
  2. When the client acknowledges ongoing instances of incest, rape, abuse of a child, adult, elderly or disabled person
  3. When the client reveals his/her intention to commit a crime or when he/she is assessed to be in danger to self, others and or society
  4. When the information is subpoenaed by the court and/or the Ministry of Social & Family Development

The counsellor is bound ethically and legally to inform the relevant authorities.

(C) FOR REMOTE COUNSELLING

  1. Please ensure that there is no one else with you
  2. Please ensure that the session is not recorded in any way

(D) RECORDING OF COUNSELLING SESSIONS

As part of our belief in ensuring quality service standards to our clients, the counselling sessions may be recorded for supervision and training purposes. Recording involves audio and video recording of counselling sessions.

  1. All recording may be used only for professional purposes of assessment, analysis, supervision and training in the interest of improving practice standards to our clients.
  2. The recording(s) may be viewed only by your counsellors, team counsellors and supervisors. Your counsellor will be responsible to ensure confidentiality and safe keeping of the tape at all times.
  3. The recording is to be erased within 3 months after the working contract with the client has ended.

*Client below 21 years of age is required to provide parent/guardian contact number.
