By completing this form, you acknowledge that the information provided will be kept strictly confidential and used solely to assess your eligibility for subsidised counselling sessions at REACH Counselling Centre @ Shunfu. Each subsidised counselling session costs $10 and lasts 1 hour in duration.
Eligibility Criteria for Subsidised Counselling
You are eligible if you have not visited our counselling centre in the past 3 months AND:
• Hold a Pioneer, Merdeka, Blue CHAS, or Orange CHAS card; OR
• Are a full-time MOE student; OR
• Are receiving ongoing Social Service Office (SSO) assistance.
Important Notes
• Parental or guardian consent is required for clients under 21 years old, and their contact details must be provided.
• Our staff will contact you within 3–5 working days with instructions on booking your counselling slot if you qualify for the subsidised counselling programme. If eligible, full payment is required to confirm your appointment.
